PRF EZ Gel Training for Providers: What You Need to Know Before Adding This Treatment

I've been performing PRF EZ Gel treatments in my Manhattan practice for years, and it's become one of the most requested regenerative treatments I offer. What Is PRF EZ Gel, and Why Are Patients Asking for It?
PRF EZ Gel is a second-generation platelet-rich product derived from the patient's own blood. Unlike traditional PRP, which is liquid and dissipates quickly, PRF EZ Gel is processed at a lower centrifuge speed without anticoagulants — which preserves more platelets, growth factors, and leukocytes. The result is a thicker, gel-like consistency that behaves similarly to a soft tissue filler, but is made entirely from the patient's own biology.

That distinction matters to today's aesthetic patient. The shift away from synthetic fillers toward biostimulatory and autologous treatments is not a trend — it's a fundamental change in what patients want. They're asking about PRF EZ Gel by name. They're coming in with screenshots from Instagram and questions about whether it's right for them. They want natural, they want longevity, and they want to avoid looking "done."

PRF EZ Gel checks all of those boxes. It's ideal for:
  • Under-eye hollows and dark circles — one of the most delicate treatment areas, where synthetic filler carries real risk of Tyndall effect
  • Nasolabial folds — softens without adding synthetic volume

Results develop over four to six weeks as collagen production is stimulated, and they continue to improve with subsequent treatments. It's not an instant result — and setting that expectation correctly is part of what good training covers.

​What Makes PRF EZ Gel Different From PRP and Standard PRF?
Providers often come to this treatment having some familiarity with PRP. It helps — but PRF EZ Gel has meaningful protocol differences that matter clinically. 

PRP is drawn with anticoagulants and spun at high speeds. The resulting product is liquid, rich in platelets, but shorter-lived once injected. It's excellent for microneedling and hair restoration, but not ideal for volumizing applications because it doesn't hold structure.

Standard PRF is drawn without anticoagulants and spun at lower speeds, preserving more of the fibrin matrix. The product is denser than PRP but still relatively fluid depending on processing.

PRF EZ Gel takes this further. By heating the PRF after centrifugation, the fibrin matrix polymerizes into a true gel — one that can be injected with a cannula or needle into areas that require both volume and biostimulation. The gel holds its position longer than liquid PRF, gives providers more predictable placement, and delivers growth factors over a more extended release period.

The centrifuge protocol — speed, time, tube type, temperature — is where most errors happen. Small variations in any of these variables produce meaningfully different products. This is why hands-on, protocol-specific training matters more for PRF EZ Gel than for almost any other treatment.

PRP vs PRF: Why One Is the More Powerful Regenerative Treatment

Both come from your own blood. Both stimulate collagen. But only one keeps working for weeks.

If you've been researching natural, regenerative skin treatments in NYC, you've probably come across two names that sound almost identical: PRP (Platelet-Rich Plasma) and PRF (Platelet-Rich Fibrin).   • A slow, sustained release. Those growth factors are released gradually over 1 to 2 weeks, not all at once.

The Vampire Facial for Melasma: What New Research Says About PRP and Microneedling

If you've been struggling with melasma, those stubborn brown or gray-brown patches that seem to darken no matter what you do, you know how frustrating this condition can be. Melasma is notoriously difficult to treat, and many patients cycle through topical creams, chemical peels, and laser treatments without finding a lasting solution.

But a new 2025 study published in the Journal of Cosmetic Dermatology offers some exciting news: PRP combined with microneedling (aka the Vampire Facial - one of my most popular treatments) showed a 73% improvement rate in melasma patients, significantly outperforming other delivery methods.

What the Study Found
Researchers conducted a randomized split-face trial on 62 patients with melasma. Each patient received PRP therapy on both sides of their face, one side treated with microneedling and the other with microinjections, over three sessions spaced one month apart.

The results were striking. Using the modified Melasma Area and Severity Index (MASI), the gold standard for measuring melasma severity, the microneedling group showed a 73.33% improvement rate, while the microinjection group showed just 18.33%.

What was even more impressive was that 46.67% of patients in the microneedling group achieved over 75% improvement (categorized as "very good response"), compared to only 5% in the microinjection group. The difference was statistically significant (p < 0.001), and both physician and patient satisfaction scores were notably higher in the microneedling group.

​Why Microneedling Makes PRP Work Better
What explains such a dramatic difference between the two delivery methods? The researchers point to microneedling's unique ability to enhance transdermal delivery. When we perform microneedling, we create thousands of tiny microchannels in the skin. These channels allow the PRP, packed with growth factors like platelet-derived growth factor (PDGF) and transforming growth factor (TGF), to penetrate deeper and more uniformly into the dermis.

This deeper, more even distribution means the growth factors can work directly on the melanocytes (pigment-producing cells) and the surrounding tissue, helping to regulate pigment production and promote healthier skin turnover. With microinjections alone, the PRP stays more localized at the injection points. Microneedling essentially turns your entire treatment area into a delivery system.

​How PRP Addresses Melasma
PRP contains concentrated growth factors that do several things relevant to melasma treatment:

- Regulate melanocyte activity: The growth factors in PRP can help normalize the overactive melanocytes responsible for excess pigment production.
- Promote skin remodeling: By stimulating collagen and elastin production, PRP helps turn over pigmented skin cells and replace them with healthier tissue.
- Reduce inflammation: Chronic low-grade inflammation can trigger melanin production. PRP's healing properties help calm the skin and reduce inflammatory triggers.
- Improve skin barrier function: A healthier skin barrier is better equipped to protect against the UV and visible light exposure that worsens melasma.

​What This Means for My NYC Patients
At my practice in the heart of Manhattan, Sarah-Kate Rems, NP, I've been offering the Vampire Facial for years to address fine lines, acne scars, large pores, and uneven skin tone. This study reinforces what I've been seeing clinically: the combination of microneedling and PRP is greater than the sum of its parts.

For melasma patients specifically, this research suggests that the Vampire Facial could be a valuable addition to a comprehensive treatment plan. I want to be clear: melasma typically requires a multi-pronged approach. That means:
  • Rigorous sun protection (SPF 50+, reapplied throughout the day)
  • Tinted sunscreen with iron oxide to block visible light

​What to Expect from Treatment
If you're considering PRP microneedling for melasma, here's what the process looks like:

The procedure: We draw a small amount of blood from your arm and process it in a centrifuge to concentrate the platelets and growth factors. Meanwhile, we apply a topical numbing cream to your face. Once you're comfortable, we perform the microneedling treatment with our SkinPen device, then apply the PRP directly to your skin so it can absorb through the microchannels.

Treatment schedule: Based on this study and my clinical experience, I recommend a series of 3 treatments spaced about 4-6 weeks apart for melasma. Maintenance treatments may be beneficial depending on your individual response.

Downtime: Most patients experience redness and mild sensitivity for 24-48 hours. You'll want to avoid direct sun exposure and wear SPF religiously during the healing period.

Results timeline: You may notice some improvement within a few weeks, but the full benefits develop over 2-3 months as your skin continues to remodel and regenerate.

Both PRP and PRF come from your own blood. The difference lies in how they're processed and what they deliver to your scalp. PRP has been the gold standard for years. It concentrates platelets and their growth factors, which stimulate dormant hair follicles. I've seen it work beautifully for patients dealing with everything from postpartum hair loss to male and female pattern thinning.

PRF takes this a step further. It's processed without anticoagulants, allowing a fibrin matrix to form naturally. This matrix acts as a scaffold that releases growth factors slowly over time, rather than all at once. The systematic review found that PRF may offer a longer-term release of growth factors compared to PRP, which could translate to more sustained results.

​What the Research Shows
The 2024 systematic review examined seven studies involving 130 patients. Three of those studies specifically focused on PRF for androgenetic alopecia (pattern hair loss), and the results were encouraging. Across the studies, the majority of patients experienced noticeable improvements in both hair density and hair growth. One study reported that 73% of patients showed clinically significant improvements after PRF treatment.

The researchers attributed these outcomes to PRF's ability to release key growth factors, including PDGF, TGF-β, and VEGF, that stimulate follicular activity and enhance blood flow to the scalp.

What I find particularly interesting is how PRF works on a cellular level. These growth factors don't just wake up dormant follicles, they help reverse the miniaturization process that causes hair to thin over time. That means PRF isn't just promoting new growth; it's helping existing hair become thicker and stronger.

​Why PRF May Have an Edge
The systematic review compared PRF to PRP and found something noteworthy: while both treatments work, PRF showed superior outcomes in certain parameters during facial rejuvenation studies, specifically because of its sustained-release mechanism. The researchers suggest this same advantage likely applies to hair restoration.

Think of it this way - PRP delivers a concentrated burst of growth factors immediately. PRF delivers those same growth factors gradually, over days or even weeks, because they're embedded in that natural fibrin matrix. For hair follicles, which need consistent stimulation to shift from dormant to active growth phases, this prolonged exposure could mean better, longer-lasting results.

The review's authors noted that PRF's unique composition "not only prolongs the bioavailability of these crucial proteins but also ensures a sustained therapeutic effect."

Based on the research and my clinical experience, PRF works best for patients with mild to moderate hair thinning - whether that's androgenetic alopecia, postpartum shedding, or stress-related hair loss.
